I thought I would live in the backcountry forever, teaching urban preteens that the forest wasn’t planted by people and our trash doesn’t just go away. When I switched to medicine in my late 20s, I thought my days dealing with air, water, and the earth were behind me. Early in my medical education, nostalgia led me to research how the environment affects health. All I had seen in class was a slide on the expansion of Lyme Disease, so that’s where I started. Within 15 min, I learned that climate change is not only worsening Lyme Disease, but also viruses, emerging fungi, waterborne bacterial illnesses, and maybe even antibiotic resistance. The Infectious Disease Society of America had published a statement about it. Every medical society had published a statement about it. This was happening now, and no one was talking about it. After moving from classroom to clinic, I began to see the environment in my patients. Mr. Williams’ aPatients have been deidentified. kidney stone didn’t just happen randomly; he was predisposed by that summer’s record-breaking heat and the urban heat island where he lived. No one could prove that pollen was the direct cause of Mrs. Johnson’s asthma attack, but a longer and stronger pollen season certainly didn’t help. The environment often wasn’t directly responsible, but it had a hand in everything. When I saw patients with COPD exacerbations, preterm births, even stroke, I couldn’t unsee how the environment intertwined with socioeconomic status to increase vulnerability. When I spoke up about what I was seeing, I learned that even some of my mentors did not understand the basics. Some told me that we need to follow the science, as if the same scientific methods that we employ in medicine had not already illuminated countless causal links between climate and health. Others told me that climate change is too political, as if avoiding human suffering were a game. Still others told me that doctors should only learn what is necessary for direct patient care, as if climate change, despite its staggering scale, were just another risk factor. Yet I remain hopeful. Environmental determinants of health are increasingly taught in medical school. My colleagues often understand that our patients’ health is determined more by where they live than by their access to care. The next generation of students and doctors is fighting against the breathtaking health impacts of climate change, and our collective voice is louder than ever. As I move on to residency, I will think back to the oath I took today. If I am to truly protect health, as I pledged, it is not enough to simply treat vulnerable patients and send them back to the fumes and heat. It is my responsibility to work to reform the systems that degrade our environment, including the healthcare system as it is today. Investments in our Earth are investments in ourselves.
