- W4382806937 abstract "Gastric ‘indefinite for neoplasia’(IFN) on endoscopic biopsy is a borderline lesion that is difficult to diagnosis as either benign or malignant. However, guidelines for management of this lesion remain unclear. Here, we aimed to evaluate the clinical course of patients with IFN on endoscopic biopsy and to determine predictive factors for progression to gastric cancer. We retrospectively included 121 cases proven as IFN on initial histology of endoscopic forceps biopsy. Of 121 cases, endoscopic submucosal dissection(ESD) (36/121, 29.8%), operation (2/121, 1.7%) and follow-up biopsy (83/121, 68.6%) were performed. The data on the initial endoscopic and histologic features of IFN lesions were analyzed to evaluate the predictive factors of neoplasms. After diagnostic modalities, the final pathologic diagnoses were cancer (38/121, 31.4 %), dysplasia (14/121, 11.6 %), and non-neoplasm (69/121, 57.0 %). In univariate analysis, lesion size of 15 mm or greater [odds ratio (OR) 3.10, 95 % confidence interval (CI) 1.17–8.12, p=0.02], lesion with mucosal unevenness (OR 4.61, 95 % CI 1.16–18.38, p=0.03), lesion with ulceration (OR 4.14, 95 % CI 1.60–10.76, p=0.004) and lesion with spontaneous bleeding (OR 4.06, 95 % CI 1.16–14.21, p=0.03) were risk factors for neoplasm. In multivariate analysis, lesion with mucosal unevenness (OR 4.29, 95 % CI 1.35–13.58, p=0.013), lesion with ulceration (OR 3.79, 95 % CI 1.37–10.51, p=0.01) and lesion with mucosal unevenness (OR 4.35, 95 % CI 1.12–8.52, p=0.03) were independent risk factors for gastric cancer. Considering the neoplastic risk of IFN, endoscopic procedures such as ESD may be more useful for definite diagnosis and treatment, particularly in cases accompanied by independent risk factors, such as lesion with mucosal unevenness, ulceration or spontaneous bleeding." @default.
