- W4382995361 abstract "Developmental pediatrics is a relatively new subspecialty in the field of pediatrics, with limited analysis to date of theoretical and conceptual issues underlying clinical practice. Developmental pediatricians assess, diagnose and help manage children and young people experiencing delays, deficits and disability in relation to their development. Families and other stakeholders seek and expect an explanation for a child's developmental differences. This chapter deals with epistemic, and inter-related ethical challenges that developmental pediatricians face as they make causal and functional attributions to explain observed behavioral phenomena. It describes and analyzes four clinical scenarios in which attribution occurs, and the challenges and issues particular to each. It argues that attribution-making as part of diagnostic practice is challenged by conceptual and terminology issues, narrow and restrictive diagnostic pathways tied to making a categorical diagnosis, and gaps in the evidence-base needed to make inferences and attributions validly and confidently. The chapter ends with reflections on ways to reduce these challenges, comprising three parts. First, revisiting and updating our conception of neurodevelopmental disorders, in light of advances in understanding of how experience affects brain development. Second, complementing categorical diagnosis-making with a broader and more holistic commitment to the process of diagnostic formulation. And finally, advocating for a scientific research agenda that can reduces sources of epistemic uncertainty through new knowledge that adds to our understanding of children's developmental trajectories and the multiple factors that influence them." @default.
