- W4383876162 abstract "Purpose Asymptomatic bacteriuria is a common finding during pregnancy. Effective antibiotic treatment could reduce its adverse effects during pregnancy. However, antimicrobial resistance limits the treatment options. Fosfomycin might be a promising drug in this regard, as its resistance is still low. The aim of the study was to determine the antimicrobial susceptibility pattern of Fosfomycin in isolates causing ASB by disc diffusion & agar dilution (in selected isolates), determine MIC by agar dilution in isolates testing resistant by disc diffusion and detect the genetic mutations responsible for fosfomycin resistance. Material & Methods This was a 2-year study (Nov. 2019 to Nov. 2021), carried in Department of Microbiology. Total 10252 urine samples from asymptomatic pregnant females (18-45 years) attending the antenatal care OPD were submitted. Identification of pathogen & AST was done as per standard methods. Phenotypic detection of Methicillin resistant S. aureus (MRSA), High level aminoglycoside resistance (HLAR), Vancomycin resistant Enterococci (VRE) & Carbapenem resistant Enterobacterales (CRE) etc. was done. Isolates were randomly tested by agar dilution. Conventional PCR was done for murA, fosA, uhpT & glpT genes on all the isolates resistant to fosfomycin by disc diffusion & agar dilution. Result In this study, the prevalence of ASB among pregnant females was around 1173(11.4%), in which E. coli 495(42%) being the predominant organism. CRE & MRSA accounted for 296(40%) & 50(66.6%) respectively. Fosfomycin was sensitive in all MRSA isolates. On agar dilution, 3 isolates showed MIC of 256µg/ml, 2 isolates showed MIC 1024µg/ml & rest with MICs >2048µg/ml. On PCR, murA & glpT genes were detected in 5 & 4 GN isolates respectively, while only 1 GP isolate was positive for fosA gene. Conclusion Our study revealed more of chromosomally mediated resistance to fosfomycin than transferrable (plasmid mediated). There was no discrepancy between the results of disk diffusion and agar dilution." @default.
