Lancet HIV. 2023; (published online July 12.)https://doi.org/10.1016/S2352-3018(23)00111-X Summary Full Text Full Text PDF PubMed Google Scholar report results of a rigorous systematic review and meta-analysis of data from 152 independent studies of gay, bisexual, and other men who have sex with men (GBMSM) conducted in 31 sub-Saharan African countries, evaluating trends in HIV testing rates, care cascade outcomes, and HIV incidence between 2003 and 2020. Findings show some progress over time. For example, HIV testing rates increased, and an estimated 82% of GBMSM in these countries had tested for HIV in the past 12 months in 2020. The authors estimated that current antiretroviral therapy (ART) use among GBMSM living with HIV increased over time and was 73% in 2020. However, only 51% of GBMSM living with HIV reported being aware of their status, creating some uncertainty in the estimate of improved ART access. In addition, viral suppression did not improve over time, with only 69% of GBMSM living with HIV estimated to have an undetectable viral load in 2020. Finally, there was no evidence of decreased HIV incidence over time, which remained disappointingly high at 6·9 per 100 person-years of observation—much higher than HIV incidence among men from the general population.
