- W4384566658 abstract "<h3>Objectives</h3> There is a recognised overlap between trauma, adverse childhood experiences and neurodevelopmental presentations.<sup>1</sup> In 2021, following a serious incident when the community paediatric team were not aware of safeguarding concerns for a child with a neurodevelopmental presentation, a new system of notification was established for the safeguarding nurses to inform community paediatricians when there is a safeguarding referral for a community paediatric patient. The purpose of this service evaluation was to analyse the safeguarding notifications made to the community paediatric team in August 2021 and June 2022, to look at the overlap between the safeguarding and the neurodevelopmental concerns of children and young people who present to both services, with a view to improve the service offered. <h3>Methods</h3> A retrospective review of the electronic patient records of the children and young people notified to the community paediatricians in August 2021 and June 2022 was undertaken. The chronologies of these cases were reviewed to look at the patient journeys, documenting their contact with services. <h3>Results</h3> 6 notifications were made in August 2021 and 12 were made in June 2022. 6 of these children had been referred to the single point of contact of the social care team, 5 had been escalated to the multi-agency safeguarding hub and 7 had been escalated to the safeguarding hub for children at risk of exploitation. A review of the chronologies of these 18 cases found there was temporal overlap between the children presenting to safeguarding services, community paediatrics for neurodevelopmental symptoms and to mental health services. The chronologies also showed that children were being referred between services and being added to multiple waiting lists, which can result in delays in the provision of support to these children with complex problems. There was evidence of families and professionals experiencing anxiety and confusion in relation to the expectations from the different referrals. The chronologies also showed that a high proportion of these children presenting with neurodevelopmental symptoms had experienced multiple adverse childhood experiences and that they were vulnerable to further exploitation perpetuating the cycle of trauma. <h3>Conclusions</h3> This service evaluation highlights the fragmented provision of services for children with neurodevelopmental conditions and safeguarding concerns. There is a need for a co-ordinated approach between social care, neurodevelopmental services and mental health services with the support of commissioners, for children presenting with overlapping safeguarding and neurodevelopmental difficulties. <h3>Reference</h3> Gajwani R, Minnis H.
