- W4384831351 abstract "Endovascular treatment with coils has become the primary treatment modality for intracranial aneurysms since Guglielmi introduced his detachable coils in the early 1990’s(1)(2). Within the past 20 years, many devices which help endovascular embolization of brain aneurysms have been shown on the market to improve the treatment of complex, wide-neck bifurcation aneurysms(3)(4). Laser-cut and braided stents are one of the most used devices in the endovascular armamentarium to perform stent-assisted coiling in X and Y-configuration for complex aneurysms(5) (6). We present a technical video for the treatment of a complex, wide-neck, non-ruptured, saccular aneurysm on the anterior communicating artery. Based on our experience and literature, the best way to save the anterior communicating (Acom) complex with minimal procedure and postprocedure complications was to perform a stent-assisted embolization with 2 parallel laser-cut (Atlas, Stryker) stents in X-configuration via left and right femoral approach. The first stent was deployed through the right A1 segment to the A2 of the left anterior cerebral artery after jailing with another microcatheter for the sequel coiling of the aneurysm. The second stent was delivered from the left A1 segment to the A2 of the right anterior cerebral artery. Both stents “kissed” in the anterior communicating artery to form good scaffolding and stability for the aneurysm coiling. The patient was premedicated with a dual antiplatelet therapy (Prasugrel 10mg and ASA 100mg per day) 5 days prior to the procedure. An assessment of the platelet function was performed the day prior to the procedure, showing excellent response (6 PRU) After the procedure, the patient has no added neurological deficit, and on the 6-month follow-up, there is no evidence of residual or recurrence of the aneurysm." @default.
