- W4385187829 abstract "Background and Aim: Intrahepatic cholangiocarcinoma(iCCA) is the second most common primary liver cancer (10-20%). It has very poor prognosis with median survival < 24 months. HBV is one of the risk factors for ICC. Recent data indicate causal role for chronic HBV infection/HBV-associated cirrhosis in the development of iCCA in HBV-endemic areas. We present a case of 56-year Male with Chronic Hepatitis B related iCCA. Case report: 56-year male presented to outpatient clinic of our hospital with abdominal pain for1 month and fever for 2 days. He was HbsAg positive. Viral load was high and started on Tab Tenofovir. His family history was significant for Chronic hepatitis B. On physical examination, no signs of liver decompensation were detected. His liver synthetic function and alpha fetoprotein value were completely normal. PIVKAII was slightly raised (48.31) Triple phase CECT abdomen showed hepatomegaly with multiple discrete and confluent altered signal lesions showing diffuse restrictions and post contrast arterial enhancement and Intraluminal tumour thrombus. PET showed a large lesion 10.3 x 14cm involving I, III, IV, VIII with enhancement in all 3 phases and perigastric, gastrohepatic, periportal lymphadenopathy. USG guided liver lesion biopsy was done which showed features of adenocarcinoma. IHC markers were sent which were positive for CK7, CK 19, Pan CK suggestive of Intrahepatic Cholangiocarcinoma. Videoendoscopy showed Grade 3 x 3 esophageal varices for which EVL done. Now the patient is planned for systemic therapy with Gemcitabine and Cisplatin. Conclusion: iCCA is devastating malignant tumor, much less common than HCC. Long term inflammatory process could contribute to HBV mediated Intrahepatic Cholangiocarcinogenesis. Hepatic progenitor cells have a role in the development of iCCA. The pathogenetic pathways through which HBV leads to the development of iCCA have been recently the targets of intense investigation. The prognosis and management is different from that of HCC." @default.
