- W4385196810 abstract "Background and Aim: Portosystemic myelopathy (PSM) is a rare complication in patients with portosystemic shunts, manifesting as pure motor spastic paraparesis. Case Report: We present a case of PSM in a patient with non-cirrhotic portal hypertension (NCPH). A 55-year-old female presented with difficulty in walking followed by progressive stiffness in both lower limbs for 6 months. There was no involvement of upper limbs. Physical examination revealed moderate splenomegaly. She had spasticity in both lower limbs (grade 3) with brisk knee and ankle jerks and bilateral extensor plantar response (Babinski sign). There were no cerebellar signs and sensory examination was normal. Higher mental functions and cranial nerve examination were normal. Laboratory investigations showed pancytopenia, normal INR, hypoalbuminemia, elevated plasma ammonia, low serum creatine kinase, normal vitamin B12 and folate levels. Cerebrospinal fluid examination and nerve conduction study were normal. Autoimmune encephalitis, immunoline, and paraneoplastic panel were negative. MRI whole spine screening was normal. MRI brain showed mild global cortical atrophy. Contrast enhanced CT scan abdomen showed dilated portal vein (17.5 mm) with tortuous porto-systemic collaterals noted in the paraduodenal and right suprarenal regions draining into right renal vein. Liver was normal. Spleen was grossly enlarged (21 cm). A diagnosis of spastic paraparesis due to portocaval shunting was considered. She was treated with intrathecal baclofen and physiotherapy. Mild improvement in spasticity was observed. PSM is an irreversible and refractory condition affecting the spinal cord usually presenting with progressive difficulty in walking and lower limb stiffness with sparing of upper limbs. Conclusion: The clinical diagnosis of PSM should be established after exclusion of other common entities affecting the spiral cord. Selective embolization or surgical occlusion of the shunt can halt disease progression and, in some cases can reverse the condition in early stages." @default.
