- W4385293399 abstract "Background: : Cerebral malaria is the most severe neurological complication of infection with Plasmodium falciparum. Malaria is the first cause of death of parasitic origin in the world. As per WHO guidelines in 2020, the region was home and malaria cases and deaths. With effective management cerebral malaria is preventable and curable. Objective: 1. To prevent the neurological complications. 2. To improve patient’s condition vitally and clinically stable. Case presentation: A 29 yrs. male admitted in A.V.B.R. hospital at MICU with chief complaints of fever, yellowish discoloration of skin, eyes since 10 days, vomiting, 5-6 episodes and loss of conscious since 2 days. Patient was brought to the hospital with above complaints in intubated state. All investigations done. Fever profile was done dengue, malaria, leptospira, scrub typhus done and was falciparum malaria positive. As patient was drowsy and had continuousness high grade fever. CSF examination showed normal cell counts. Hb 6.9 gm%. Transfused with 1 unit of packed red cells. Daily monitoring of CBC, KFT, LFT, and urine output observed. Inj. Larinate 120mg. Inj.Doxy 100mg, inj. Meropenem500 mg, inj. Piptaz 2.25gm, Inj.Lasix 40mg, Inj.Emset 4mg, Inj. Pan 40, Dextrose 25% given. As patient’s condition was improved, ventilator was weaned off & was taken of BIPAP & later on oxygen support. In view of deranged KFT, urea- 44 mg/dl, creatinine- 6.9 mg/dl. Nephrologist opinion was taken, they advise acute kidney injury. And Serial hemodialysis done. Daily CBC, KFT, was monitored. KFT shows decreasing ranged urea 25 mg/dl and creatinine 2.9 mg/dl. During course in the hospital, patient’s condition improved gradually and responded to treatment. Prognosis: Patient’s duration of disease & hospitalization was 28 days. Patient was discharged after found vitally and clinically stable. Conclusion: Primary managing entails correct diagnosis, lowest chance of morbidity and mortality." @default.
