- W4385408780 abstract "Staffing instability is weakly correlated with staffing turnover in nursing homes, but each is independently associated with nursing home quality, a cross-sectional study found. Led by Soham Sinha, a PhD student at the University of Chicago, researchers examined one year of administrative data for 11,840 nursing homes, merging data about daily staffing from the Payroll-Based Journal and data about nursing home characteristics, total staff turnover, and nursing home quality from the Nursing Home Care Compare report card. The researchers defined staffing instability as when “nurse staffing is at least 20 percent less than that facility’s mean staffing, adjusted for daily resident census.” They defined turnover as “the percentage of nursing staff that left the nursing home over a 12-month period.” Staffing instability was associated with lower levels of activities of daily living, increased prescriptions of antipsychotic drugs among long-stay residents, and a decline in function at discharge in short-stay residents. Turnover appeared to have more of an impact on resident health care, including a higher prevalence of pressure ulcers and declining mobility for long-stay residents and more hospitalizations for short-stay residents. As might be expected, facilities with high turnover and high instability also had fewer hours per resident for nursing staff than the facilities with lower turnover and instability. Also, facilities with high turnover and instability on average had lower overall ratings, lower staffing ratings, and lower quality measure ratings.
