Primary statistical data were obtained from reports of chief surgeons in 18 regions of the Central Federal District of the Russian Federation and presented in analytical collections «Surgical care in the Russian Federation».The number of patients admitted to surgical hospitals of the Central Federal District with acute abdominal diseases later than 24 hours from clinical manifestation varies depending on the underlying disease. The greatest number of late hospitalizations was observed in acute intestinal obstruction (50.82%), acute adhesive intestinal obstruction (48.49%) and acute pancreatitis (47.36%). In acute cholecystitis, gastrointestinal bleeding and acute appendicitis, admission after 24 hours was observed in 44.72, 38.65 and 33.83% of cases, respectively. Late hospitalization is even less typical for strangulated hernia (27.43%) and perforated ulcer (26.23%). In-hospital mortality significantly differs in both groups (within and after 24 hours) for all acute abdominal diseases. Extended surgery and widespread peritonitis increase these differences for strangulated hernia by 9.2 times (0.92% within 24 hours and 8.48% after 24 hours), for acute appendicitis by 8 times (0.05% within 24 hours and 0.40% after 24 hours) and for perforated ulcer by 6.3 times (4.50% within 24 hours and 28.59% after 24 hours).In the Central Federal District, about 25-50% of patients with acute abdominal diseases admitted to the hospital later than 24 hours after clinical manifestation depending on disease. We found the highest in-hospital mortality following late hospitalization in patients with strangulated hernia, acute appendicitis and perforated ulcers.Сравнить влияние сроков госпитализации пациентов на уровень госпитальной летальности при острых абдоминальных заболеваниях в Центральном федеральном округе Российской Федерации.Анализ поздней госпитализации и госпитальной летальности проведен на основании метаданных (616 742 клинических наблюдения за 2017—2021 гг.), созданных по первичным статистическим сведениям из отчетов главных хирургов 18 регионов ЦФО РФ, которые представлены в информационно-аналитических сборниках «Хирургическая помощь в Российской Федерации».Число пациентов, поступающих в хирургические стационары ЦФО с острыми абдоминальными заболеваниями позже 24 часов от начала заболевания, варьирует в зависимости от характера основного заболевания. Наибольшее количество наблюдений поздней госпитализации отмечено при острой кишечной непроходимости (50,82%), острой спаечной кишечной непроходимости (48,49%) и остром панкреатите (47,36%). При остром холецистите, желудочно-кишечном кровотечении язвенной этиологии и остром аппендиците поступление позже 24 часов имело место в 44,72%, 38,65% и 33,83% соответственно. Еще реже поздняя госпитализация характерна для ущемленной грыжи (27,43%) и прободной язвы (26,23%). Госпитальная летальность значимо различается в группах сравнения (до и позже 24 часов) при всех острых абдоминальных заболеваниях. Расширение объема оперативного вмешательства и развитие распространенного перитонита обуславливают наибольшую выраженность этих различий при ущемленной грыже в 9,2 раза чаще (0,92% — до 24 часов и 8,48% — позже 24 часов), у пациентов с острым аппендицитом — в 8 раз чаще (0,05% — до 24 часов и 0,40% — позже 24 часов) и с прободной язвой — в 6,3 раза чаще (4,50% — до 24 часов и 28,59% — позже 24 часов).В зависимости от характера основного патологического процесса, от четверти до половины пациентов с острыми абдоминальными заболеваниями в ЦФО поступают в стационар позже 24 часов от начала клинической симптоматики. При сравнительном анализе наибольший рост госпитальной летальности при поздней госпитализации наблюдали у пациентов с ущемленной грыжей, острым аппендицитом и прободной язвой." @default.
