- W4385523003 abstract "I agree completely with Dr. Hsu that transtibial pull-out repair with absorbable sutures is not an ideal method for fixation of the meniscus root.1Hsu J.C. Regarding “Outcomes of arthroscopic all-inside repair are improved compared to transtibial pull-out repair of medial meniscus posterior root tears.”.Arthroscopy. 2023; 39: 1957-1958Abstract Full Text Full Text PDF Scopus (1) Google Scholar Various materials have been used in repairing medial meniscus posterior root tears, including absorbable polydioxanone, nonabsorbable polyester, and high-strength sutures made of ultrahigh-molecular-weight polyethylene (UHMWPE). The introduction of UHMWPE-incorporating polyblend sutures has improved suture material design. These sutures are stronger and stiffer than conventional ones, making them suitable for repairs.2Ahn J.H. Wang J.H. Arthroscopic medial meniscus posterior root fixation using a modified Mason-Allen stitch.Arthrosc Tech. 2016; 5: e63-e66Abstract Full Text Full Text PDF PubMed Scopus (17) Google Scholar The main purpose of the study was not to discredit the effectiveness of transtibial pull-out repair. Rather, the objective of this study was to investigate the efficacy of all-inside repair of medial meniscus posterior root tears. As Dragoo et al.7Dragoo J.L. Outcomes of arthroscopic all-inside repair vs observation in older patients with meniscus root tears: Response.Am J Sports Med. 2020; 48: Np50-Np51Crossref Scopus (1) Google Scholar,8Dragoo J.L. It also minimizes the surgical time because it does not require any special instruments such as transtibial drill guides. The only necessary tool is an all-inside meniscal repair device.7Dragoo J.L. This study was a retrospective study with small number of patients. Further clinical and biomechanical trials is requisite to gain more evidence for the technique described. In addition, they concluded that the all-inside repair technique is a viable treatment option based on favorable outcomes compared with a historical transtibial pullout repair group.
