- W4385525070 abstract "Resuscitative thoracotomy is often required when patients arrive at a trauma center “in extremis,” impending cardiopulmonary arrest, or in arrest. Patients arrest when they have lost approximately 50% of blood volume. It is also indicated when patients arrive in profound shock, particularly when they sustain penetrating thoracic injuries. It can also be employed in very selected patients presenting with abdominal vascular and peripheral vascular injuries. Patients requiring this procedure incur high rates of mortality. Emergency department thoracotomy (EDT) remains a formidable tool within the trauma surgeon’s armamentarium. Since its inception in 1960 it has been employed at times for vague indications, although this life-saving procedure is most often indicated for patients that sustain penetrating thoracic injuries, with the best survival achieved for patients with penetrating cardiac injuries. Asensio, along with Wall, Minei, and others defined the indications for its performance along with survival rates stratified to injury mechanisms; they also described the poor neurologic outcomes for patients undergoing this procedure for blunt trauma. This chapter reviews the history, technique, outcomes, and literature for this technically difficult but life-saving procedure." @default.
