- W4385568763 abstract "Pancreatic neuroendocrine tumors (PanNET) have become more frequent worldwide. By the time of diagnosis, up to 40% of PanNETs have spread to distant organs. Approximately 10% of PanNET are inherited. These patients typically have multiple lesions scattered throughout the pancreas and necessitate more customized treatment plans than are required for solitary sporadic PanNET. Given the unavailability of equally effective nonsurgical treatment options, resection of a locally advanced nonfunctioning PanNET at institutions experienced in pancreatic surgery is the method of choice. Major pancreatic resection in advanced PanNET frequently includes the spleen (>70%), but also major vessels (17%), stomach (4%), bowel (2%), and kidney (2%). Retrospective data suggest that complication rates after multivisceral resection are similar when compared to standard surgery. Although resection of the primary tumor, even when metastases are present, may prolong survival, the potential morbidity inherent in major pancreatic resection must be weighed against the benefits. Especially for older patients (>70 years), there is conclusive evidence that advanced tumor resection does not improve cancer-specific survival. Neoadjuvant treatment regimens, including polychemotherapy or peptide receptor therapy, can produce tumor shrinkage, enabling resection of previously unresectable primary tumors." @default.
