• W4385568765 abstract "Increasing attention has been paid to cystic neoplasms of the pancreas over the last decade. With the use of modern imaging techniques, a growing number of these lesions are detected incidentally and our knowledge of pancreatic cystic neoplasms (PCNs) has increased rapidly. The most frequently resected and clinically relevant entities include intraductal papillary mucinous neoplasms (IPMNs), mucinous cystic neoplasms (MCNs), serous cystic neoplasms (SCNs), and solid-pseudopapillary neoplasms (SPNs). They differ in incidence, localization, age and sex correlation, and show specific gene alterations. Some of these neoplasms have the potential to progress and are precursors of invasive pancreatic ductal adenocarcinoma (PDAC). IPMNs are the most common PCN and can be divided into main-duct, branch-duct, and mixed-type IPMNs. Main-duct and mixed-type IPMNs harbor a risk for invasive carcinoma of about 11–81%, whereas branch-duct IPMNs reveal invasive cancer in approximately 6–38%. A time delay of circa five years is estimated between the occurrence of IPMNs with low-grade dysplasia and IPMNs with invasive carcinoma, but malignant transformation is still not well understood. MCNs are encapsulated and typically composed of mucin-producing epithelial cells and an ovarian-like stroma. Approximately 30% of resected MCNs reveal an associated invasive carcinoma at the time of resection. Presuming resection in a premalignant stage, MCNs result in cure without relevant risk of recurrence. SCNs are benign, slow-growing tumors that predominantly affect females. They are composed of a cuboidal glycogen-rich epithelium. SPNs have an incidence of 1–2% of all PCNs and occur predominantly in young women. SPNs range from almost entirely cystic to mostly solid forms, lacking a fibrous capsule. They are potentially malignant with a relevant risk of local recurrence or metastatic spread after resection.
