Diet has a prominent role in the treatment of irritable bowel syndrome (IBS); however, there is no consensus on what diet is optimal. Identifying dietary triggers that could provoke or exacerbate gastrointestinal symptoms is essential to managing IBS. We evaluated the therapeutic effect of a personalized dietary intervention on clinical outcomes in IBS. Adults (n=16) with a physician’s diagnosis of IBS were included in a 12-week prospective, single-arm study (NCT05178017). Blood specimens were collected to perform the in-vitro Leukocyte Activation Assay-MRT (LAA-MRT®) using Sony EC 800 Dual-Mode Flow Cytometry Systems. The personalized oligoantigenic dietary approach was implemented in phases by eliminating moderate- to high-immune-reactive foods and chemicals determined by the Leukocyte Reactivity Index (LRI). A registered dietitian nutritionist (RDN) administered the dietary intervention virtually. Gastrointestinal symptoms severity and frequency were measured using the IBS-Symptom Severity Scale (IBS-SSS), IBS-Global Improvement Scale (IBS-GIS), and Gastrointestinal Symptom Survey (GISS). An independent Institutional Review Board (IRB) approved the study. The majority of participants (n=14, 87.5%) were women and had a mean [SD] age of 53.7 [18.9] years and a BMI of 28.7 [7.6] kg/m2. There were significant improvements pre-post dietary intervention in gastrointestinal symptom scores: IBS-SSS (279.4 [66.2] vs. 111.2 [82.7], p<0.001); IBS-GIS (4.2 [1.4] vs. 2.3 [1.7], p=0.011); and GISS (16.9 [6.9] vs. 4.9 [4.2], p<0.001). Personalized dietary modifications guided by the LAA-MRT® represent an effective approach for managing IBS-related symptoms.
