- W4386424946 abstract "過活動膀胱は尿意切迫感を必須とし，頻尿，夜間頻尿を伴い，切迫性尿失禁の有無は問わないと国際禁制学会により定義されている．本邦における過活動膀胱患者は約1,000万人以上と推定され，生活の質の低下が問題となっている．過活動膀胱に対する薬物治療としては，抗コリン薬，β3受容体作動薬の服用ならびにボツリヌス毒素膀胱内注入療法などが挙げられる．しかしながら，いまだ薬物が奏功しない，もしくは有害事象により治療を中断する例は一定数存在する．そして，現行の薬物療法は膀胱を中心とした末梢臓器に作用する薬物であり，排尿および蓄尿を制御する脳に作用する効果的な薬物は存在しない．アンジオテンシンⅡ（Ang II）は末梢のみならず中枢レベルにおいても，血圧および水分調整など様々な生理反応に関与することが報告されている．これまで筆者らは，脳内Ang IIが排尿反射を促進する生理作用を有することを動物実験にて報告した．そして，その反応はAng IIタイプ1（AT1）受容体下流シグナル経路（phospholipase C/protein kinase C/NADPH oxidase/superoxide anion）活性化ならびにGABA神経系抑制を介することを提起した．さらに，中枢移行性のある降圧薬AT1受容体拮抗薬（ARB）テルミサルタンの経口投与が脳内Ang IIによる頻尿を抑制した．これら結果より，高血圧かつ過活動膀胱に対して，ARBは両疾患を抑制する有効な薬物である可能性が示唆された．本稿では，脳内AT1受容体を標的としたARBの頻尿抑制効果について，著者らの研究成果を中心に紹介する．" @default.
