- W4386444054 abstract "Arthrocentesis is an effective treatment method for patients with sudden and severe closed lock. Insertion of needles into the superior joint space of the temporomandibular joint (TMJ) under local anesthesia is possible. Patients can benefit from pain reduction and immediate increasing of jaw mobility. Arthrocentesis has been considered to be a noninvasive surgical option before surgical intervention to treat TMJ internal derangement not improved by conservative non-surgical therapies. The mechanisms of arthrocentesis are: 1) Removing adhesions between the disc surface and the mandibular fossa, 2) Cleaning inflammation- and pain-producing materials, and 3) Changing intra-articular pressure. Osteoarthritis, also known as degenerative joint disease (DJD), is a joint disorder. The most common cause of osteoarthritis is overload in joint tissue which occurs when the articular surface is damaged by dislocation of joint and retrodiscitis. Pain may occur when bone changes actively occur. Osteoarthritis patients experience joint pain at affected area, mouth opening is limited, and crepitus sounds are often heard. Usually, condyle at subarticular bone of the mandible appear flattening, heterotopic bone formation and erosion. At the Department of Oral and Maxillofacial Surgery, Gangnam Severance Hospital, single puncture arthrocentesis is done to TMJ osteoarthritis patients for reducing TMJ adhesions, pain control and improving mouth opening. The purpose of this study is to find the effect of arthrocentesis on TMJ osteoarthritis patients. Materials and methods:This retrospective study included 32 patients with TMJ Osteoarthritis who had undergone arthrocentesis at the OMS clinic at Gangnam Severance Dental Hospital from March 2016 to August 2022. Arthrocentesis was performed with a single puncture technique. After sanitizing the skin surface with povidone, followed by local anesthesia using 1:100,000 lidocaine/epinephrine, arthrocentesis was performed using Guardix (hyaluronic acid). Statistical analysis was performed using SPSS 25 (IBM Corp, Armonk, NY). Reliability and comparative analyses were conducted according to groups. We collect data about maximum mouth opening (MMO) before arthrocentesis, passive MMO with manipulation right after arthrocentesis, and MMO after 1, 3, and 6 months of arthrocentesis." @default.
