- W4386477046 abstract "Background: Late gadolinium enhancement (LGE) detected by cardiac magnetic resonance imaging (MRI) has been shown to be a risk factor for ventricular arrhythmias (VAs) in hypertrophic cardiomyopathy (HCM) patients. Mechanical dispersion (MD) detected using speckle tracking echocardiography may serve as a surrogate indicator to stratify risk for VA and sudden cardiac death (SCD). Aims and Objectives: To study the relationship between global longitudinal strain (GLS), MD, and relation to VAs in HCM patients. Materials and Methods: This was a prospective study of 56 patients with hypertrophic cardiomyopathy. In total, 42 patients were in HCM without the VA group, and 14 patients were in HCM with the VA group. MRI was done on 44 patients. Results: GLS in HCM patients without VA was -17.2 ± 4.6, and HCM with VA was -15.6 ± 3.5 (P = 0.249). MD in HCM patients without VA was 41 ± 26 ms, and HCM with VA was 51 ± 39 ms (P = 0.249). LGE in MRI was in 72% of patients (24 out of 33) in HCM without VA group and 100% of patients (11 out of 11) in HCM with VA group (P = 0.052). LGE volume % in HCM with VA was 11% (5.9–14.2), and HCM without VA was 11% (7.18–17), with no statistically significant difference between groups (P = 0.581). European Society of Cardiology 5-year SCD risk score was 3.1 in HCM without VA and 6.6 in HCM with VA (P = 0.0001); out of American heart of cardiology/American heart association risk factors for SCD in HCM, unexplained syncope was seen in 11 (26%) patients in HCM without VA and 8 (57%) patients HCM with VA (P = 0.034). Conclusion: We did not find a statistically significant correlation between GLS, MD, and risk of VA in our study." @default.
