- W4386630141 abstract "To compare the accuracy of ultrasonography (US) and MRI in antenatal diagnosis of placenta accreta (PA). To specify the most common US signs associated with the diagnosis of PA. A prospective study in a high-risk patients of PA admitted from 01/2014 to 12/2015 in a level II maternity unit in Tunisia. All the patients beneficiated antenatally from US and MRI. Parameters assessed during US examination included loss of retroplacental clear zone, irregularity and width of uterine-bladder interface, smallest myometrial thickness, presence of suspect lacunar spaces, and bridging vessels. MRI signs assessed were: abnormal uterine bulging, increased vascularity, heterogeneous signal of the placenta on T2-weighed, interrupted bladder line, and smallest myometrial thickness. Diagnosis of PA was confirmed histologically after hysterectomy or clinically in case of successful conservative treatment. Statistical analyses were performed X square tests and receiver operating characteristic curves. Of 10 gravidas who met inclusion criteria, 6 (60%) had invasion confirmed on hysterectomy specimen. The most relevant signs associated with diagnosis of placenta accreta in US were: smallest myometrial thickness (sensibility 0.83/specificity 0.75), abnormal placental lacunae (sensibility 0.66/specificity 1.0) and irregularity of uterine-bladder interface (sensibility 0.33/ specificity 1.0). The most relevant signs in MRI were: smallest myometrial thickness (sensibility 0.75/specificity 0.66) abnormal uterine bulging (sensitivity 0.55/specificity 0.66), and placental heterogeneity (sensitivity 0.5/specificity 0.6). Association of three US signs had the best sensitivity/specificity ratio 0.83/ 0.75 with AUC = 0.896. Using MRI, the best ratio was obtained with the association of 2 signs: 0.75/0.33 with an AUC = 0.792. Ultrasonography is the first in line exam to be used in high-risk population. Use of US indexes for the diagnosis of placenta accreta including at least three signs could be helpful." @default.
