- W4387063211 abstract "Clinical Case AbstractUnresolved catching sensation of the right Knee Taofeek Akinpelu, WellSpan Health Sports Medicine Program, York PA HISTORY: This is a case of a 38-year-old female with significant past medical history of controlled TIDM, hypothyroidism who was evaluated for chronic right knee pain. She has history of meniscal repair of the right knee when she was 15 years old. However, she had been pain-free, until approximately a year ago when she has been having progressively worsening anterior knee pain. She described constant sharp, non-radiating pain located in the anterior infrapatellar region. Pain is worse with prolonged standing. She denies any recent trauma or inciting event. She was initially diagnosed with patellofemoral pain syndrome, as initial exam was significant for hip weakness, tight hamstrings and iliotibial band. Despite compliance to prescription antiinflammatory & physical therapy, she notes persistent popping sensation and anterior knee pain. She denies paresthesia, numbness or muscle atrophy. PHYSICAL EXAM: Patient appeared to be in no acute distress. Right knee exam showed no gross deformity or joint effusion. There is tenderness to palpation of the proximal pole of the patella, however no tenderness elicited over patellar tendon, tibial tubercle, medial or lateral joint line. ROM limited to 0-120° on right knee vs 0- 180° on the contralateral side. The Lachman test, anterior drawer, pivot shift test, varus/valgus instability and McMurray test were negative. DIFFERENTIAL DIAGNOSIS: •Patellofemoral pain syndrome•Patellar tendinosis •Quadriceps tendinosis •Fat pad impingement syndrome•Plica syndrome TEST RESULTS: Right knee X-Ray-No focal fracture, subluxation, dislocation. MRI of the right knee-Hypertrophy within the synovium at the quadriceps attachment to the patella consistent with quadriceps impingement. Normal quadriceps tendon and patella tendon, without tear or tendinosis. FINAL DIAGNOSIS: Quadriceps fat pad impingement TREATMENT & OUTCOME: •Oral NSAIDs•Physical therapy with emphasis on quadriceps strengthening•Quadriceps fat pad corticosteroid injection•Arthroscopy with fat pad excision, debridement of hypertrophic fibrosis and anterior interval release•Continuation of physical therapy•She endorsed symptom improvement at 6 weeks post op visit." @default.
