METHODS: We used electronic health record (EHR) data from a large Northern California health plan to compare smoking status among Chinese men with English (EP, N=27,863) vs. Chinese (CP, N=16,831) spoken language preference and by Cantonese (N=11,207) vs. Mandarin (N=6369) dialect in 2019. Current and ever smoking status was based on EHR data. Comparisons within age groups were also conducted: 25-34y, 35-44y, 45-54y, 55-64y, 75-84y. Prevalence estimates were reported with 95% confidence intervals (CI). Modified log-Poisson regression was used to examine age-adjusted prevalence ratios (aPRs) by preferred language. RESULTS: Current and ever smoking prevalence among 44,694 Chinese men aged 25-84y was 9.3% and 30.7%, respectively, and was higher among men with CP vs. EP (16.4% vs. 5.0% and 42.7% vs. 23.5%). The CP (vs EP) aPR for current and ever smoking was 3.68 (CI 3.46-3.91) and 1.75 (1.70-1.80), respectively. Across all age groups, current smoking prevalence was higher among men with CP vs. EP: 17.7% vs. 6.3% (25-34y), 22.0% vs. 7.3% (35-44y), 23.5% vs. 5.0% (45-54y), 20.1% vs. 5.2% (55-64y), 10.4% vs. 3.1% (65-74y), and 3.8% vs. 2.1% (75-84y). A similar pattern was seen for ever smoking: 26.8% vs. 17.7% (25-34y), 37.6% vs. 23.5% (35-44y), 43.3% vs. 19.5% (45-54y), 46.9% vs. 23.9% (55-64y), 44.9% vs. 26.2% (65-74y), and 40.0% vs. 32.4% (75-84y). Among men aged 25-84y, current smoking prevalence differed slightly for Cantonese (16.5%) compared to Mandarin (15.3%) subgroups, and ever smoking prevalence was also higher in the Cantonese subgroup (44.7% vs. 38.1%). The Cantonese (vs. Mandarin) aPR was 1.20 (1.11-1.28) for current smoking and 1.15 (1.11-1.20) for ever smoking. Current smoking prevalence was higher in Cantonese vs. Mandarin subgroups for ages 35-44y (24.4% vs. 18.3%) and 45-54y (24.9% vs. 20.5%) and ever smoking prevalence was higher in Cantonese vs. Mandarin subgroups for ages 35-44y (40.3% vs. 34.0%), 45-54y (45.4% vs. 38.8%), 55-64y (48.4% vs. 43.0%), 65-74y (45.9% vs. 40.0%), and 75-84y (42.0% vs. 35.2%). CONCLUSIONS: Our findings in US Chinese men support survey-based studies reporting higher smoking prevalence among limited English proficient vs. English proficient Asian men. However, overall estimates of current smoking prevalence mask differences by age group and Chinese dialect. CLINICAL IMPLICATIONS: Disaggregation of smoking data among heterogeneous Asian populations remains important. Dialect subgroups within Chinese male populations can represent sociodemographic and culturally distinct populations, including different immigration periods. The higher prevalence of ever smoking in men with Cantonese dialect preference underscores the need for dialect-specific outreach for smoking cessation and lung cancer screening.
