- W4387247935 abstract "SESSION TITLE: Pulmonary Manifestations of Systemic Disease Case Report Posters 13 SESSION TYPE: Case Report Posters PRESENTED ON: 10/10/2023 09:40 am - 10:25 am INTRODUCTION: Although Crohn's Disease (CD) is systemic, the pulmonary manifestation of CD is rare. Bronchopulmonary manifestations are about 0.4%; even less often is it seen to cause a pneumothorax. CASE PRESENTATION: A 24-year-old female with history of Crohn's Disease status post hemicolectomy, 2 previous pneumothoraxes' status post wedge resection and pleurodesis who presents with chief complaint of sudden onset of chest pain and dyspnea. She is an ICU RN doing chest compressions on a patient, during which she suddenly became short of breath with associated left-sided chest pain. In the ED, she was found to be tachypneic and hypoxic and was placed on supplemental oxygen via nasal cannula. A CT Chest was done that was consistent with a small left sided pneumothorax. The patient was weaned off oxygen, her pain was controlled, and was able to ambulate independently. Final chest x-ray showed stable pneumothorax. She was cleared for discharge by cardiothoracic surgery with outpatient follow-up with pulmonology. DISCUSSION: Extraintestinal manifestations of CD with involvement of the lungs are exceedingly rare. However, the embryological origin of both the lungs and gastrointestinal track are from the foregut. Which is the proposed etiology of the development of granulomatous evidence in the lungs. In addition to the similar immune complexes that circulate in both organ systems. CONCLUSIONS: Recognizing pulmonary involvement in CD, particularly in those with sequala such as in our patient with recurrent pneumothorax without any other clear cause is a possibility clinicians should consider. Raising awareness of this rare manifestation of CD will help prevent the delay in diagnosis and prevent extensive unnecessary work-up for the patient.
