Unlike viral croup, recurrent croup describes a condition that may have multiple underlying causes, with attributions made to asthma, allergy, eosinophilic esophagitis, gastroesophageal reflux and anatomic airway abnormalities. While some institutions in Canada have site specific strategies, there remains a lack of consensus on management and no published treatment algorithms are in use in Canada. METHODS: We conducted a nationwide survey of paediatric respirologists and otolaryngologists in a web-based format (Qualtrics), with questions regarding centre and individual management practices for recurrent croup. In addition, a retrospective analysis was performed on croup diagnosis, management, and outcomes. The analysis was performed for patients from a multi-disciplinary joint-airway clinic at London Health Sciences Centre, in London, Ontario, Canada over the past 10 years. Descriptive statistics was performed for the following findings. RESULTS: A total of 72 survey participants from over 13 institutions across Canada responded. Among those surveyed, 62% were reported as paediatric respirologists and 38% as otolaryngologists, with a majority having 10 or more years of clinical experience in healthcare. Recurrent croup was most commonly defined as having more than three episodes per year, and the median referral to otolaryngology occurred after three episodes. Otolaryngology (46%), respirology (32%), and pediatricians (21%) primarily managed recurrent croup, while gastroenterology was less commonly involved (2%). Only 21% of individuals reported having a multi-disciplinary team to manage recurrent croup patients. Standing order prescriptions for oral steroids were divided, with 51% reporting doing so and 49% not. When prescribed, dexamethasone 0.6mg/kg was the most common dose given once. Most institutions did not have a standardized approach for nebulized or metered dose inhalers, and anti-reflux medication was uncommonly used (23%). Investigations were primarily nasopharyngoscopy (34%), bronchoscopy (26%), and X-ray (26%), with CT being less common (7%). Airway evaluation in the OR was indicated in those with severe, refractory, and symptomatic recurrent croup, and was performed in collaboration with ENT. Direct laryngoscopy or flex and rigid bronchoscopy were equally common investigations. Airway evaluation when normal was usually managed with reassurance, while subglottic stenosis grade 1 was the most common abnormal finding followed by tracheomalacia. In subglottic stenosis grade 1, most reported managing the patient through observation (68%), with medical treatment (26%) and surgery (6%) being reserved based on the severity of symptoms. CONCLUSIONS: There remains significant disparities regarding management of recurrent croup across Canada. This study elucidates some of the common diagnostic and management approaches from clinicians nationwide. CLINICAL IMPLICATIONS: These findings will help support the development of consensus positions that would standardize treatment of this common and severe disease.
