There are 2.55 million U.S. middle and high school students currently using e-cigarettes. The safety of e-cigarette consumption (vaping) remains controversial due to limited evidence. Moreover, the heating process itself can form compounds of questionable toxicity. Because e-cigarettes have only been marketed for ∼10 years, the potential long-term effects of e-cigarette consumption have been scarcely investigated. However, harmful effects of short-term e-cigarette has already been described. This data provides valuable insight into the pathophysiology of e-cigarette use and foreshadows what long term data will reveal. Below we present a case of bilateral spontaneous pneumothoraxes requiring VATS in a young female with no risk factors other than a history of vaping. CASE PRESENTATION: A 14-year-old female with no significant past medical history presents to the ER with acute onset right sided chest pain. She had no clinical features of Marfan's syndrome. Her social history was significant for vaping. Vitals signs on admission were within normal limits. Initial Chest x-ray revealed a right apical pneumothorax measuring 19 mm. Chest x-ray in the morning revealed progression of the pneumothorax to 35 mm, so a chest tube was placed. The pneumothorax did not resolve and the patient was taken to the operating room (OR) for Video-assisted thoracoscopic surgery (VATS) with wedge resection of ruptured bleb from apex of right upper lobe, parietal pleurectomy, and intercostal nerve block. She was discharged. She was later admitted at 17 years old for a large left sided pneumothorax approximately 35 mm. Her presentation was similar but this time with left-sided chest pain. Again, her only risk factor was that she was still vaping. She required VATS with apical wedge resection and mechanical pleurodesis to resolve her pneumothorax. DISCUSSION: The National Academy of Sciences, Engineering, and Medicine (NAS) concluded that there is not adequate research to predict long-term health outcomes of vaping. Additionally reporting there is sufficient evidence that use of e-cigarettes can cause acute endothelial dysfunction, oxidative stress, symptoms of dependence, increased heart rate, DNA damage and mutagenesis. It is essential to educate the public on pathologic respiratory effects of e-cigarette use, particularly for adolescents. One such short term pathology is the growing body of literature demonstrating the correlation between vaping and spontaneous pneumothorax. Even more concerning is that in many cases surgical intervention has been required. CONCLUSIONS: Bilateral spontaneous pneumothorax associated with vaping has scarcely been described in literature and this case highlights the severity of the respiratory pathology that can be seen in otherwise healthy individuals.
