• W4387248068 abstract "SESSION TITLE: Procedures Case Report Posters 2 SESSION TYPE: Case Report Posters PRESENTED ON: 10/11/2023 12:00 pm - 12:45 pm INTRODUCTION: Tracheobronchial stenting is an integral treatment modality for patients with tracheal or mediastinal lesions causing obstruction of the airway. The aim of treatment is either for palliative purposes or to help afford definitive therapy for their disease. The Y-stent extends from the distal trachea into the proximal mainstem bronchi allowing for air to flow freely past regions of obstruction. CASE PRESENTATION: This case reviews a 65 year old male with 40 pack year smoking history presented with six months of cough and dyspnea. CT chest identified a large poorly defined infiltrating malignant mass in the left perihilar region. Rigid bronchoscopy revealed the main carina was obliterated by squamous cell carcinoma extending from the left mainstem bronchus to the right main stem (RMS). RMS was opened up with tumor debulking. A fully covered metallic 12mm x 40mm bronchial stent was placed in the right mainstem bronchus and discharged home with outpatient follow up.He presented to the ED 6 weeks later with suspected pneumonia. Given acute infection, the RMB stent was removed and copious mucopurulent secretions were suctioned. Using a rigid bronchoscope a 12mm (left- and right mainstem) x 16mm x 40mm fully covered metallic Y-stent was placed without complication. Notably, this was the first fully covered metallic Y-stent placed in Chicago, IL. Patient remained intubated for 24 hours following the procedure before extubation in the ICU and subsequent discharge home. Patient was planned to continue his chemo therapy and was planned to have the stent removed once patient has finished his therapy and the left mas had regressed. DISCUSSION: Malignant central airway obstructions are life threatening. Stenting can secure airways and allow patient to be able to get their definitive treatment. Y-stent placement should be considered for salvage and palliative therapy in patients with mainstem bronchus masses causing malignant central airway obstruction. CONCLUSIONS: The new fully covered metallic Y-stents hold considerable promise in treating central airway obstructions.
