The prognosis for cancer of the right middle lobe has not been definitively established, but prior studies have suggested that tumor primary site should be considered in treatment management and prognosis assessment. In this study, we evaluated the association between the primary tumor location and prognosis in all lung cancer patients based on SEER database. METHODS: Using the Surveillance, Epidemiology, and End Results (SEER) database, Lung cancer patients diagnosed between 1999 and 2019 were selected to identify prognosis according to isolated primary sites of lesion, including the upper lobe, middle lobe, lower lobe, and main bronchus lesion of the lung. Kaplan-Meier methods and log-rank tests were used for survival comparisons while multivariable Cox regression models were used to analyze the influence of sex, tumor grade, and staging on overall survival. RESULTS: Data from 779,654 eligible patients were extracted from the SEER database. Among the patients, 40,111 had middle lobe as a primary tumor site, followed by 459,833 in the upper lobe, 235,556 in the lower lobe, and 44,154 in the main bronchus. Patients with main bronchial tumors had the worst prognosis than tumors at other locations (p<0.001) followed by upper and lower lobes with a similar 5-year overall survival rate of 19.5%. This is contrasted by 22.5% for the middle lobe tumors which showed the best 5-year overall survival outcomes among other locations. Multivariate analysis showed that sex, tumor grade, and tumor stage were all significant predictors of survival. For sex, male patients had worse survival outcomes than females. For tumor grade and staging, patients with poorly differentiated and higher-staged tumors had worse outcomes than well-differentiated and lower-staged tumors. CONCLUSIONS: The study showed that different primary tumor sites affect survival outcomes in lung cancer patients with primary tumors located in the right middle lobe having the best 5-year overall survival rate. This is independently affected by sex, tumor grade, and tumor staging as evidenced by the multivariate analysis. CLINICAL IMPLICATIONS: Regrettably, the histological data was deemed unsuitable for analysis due to insufficient data collection. Other limitations of the study included the lack of information about the treatment approach, which is of great importance, particularly considering the variations in primary tumor locations.
