- W4387248952 abstract "SESSION TITLE: Asthma Treatment and Longitudinal Studies SESSION TYPE: Rapid Fire Original Inv PRESENTED ON: 10/09/2023 12:00 pm - 12:45 pm PURPOSE: The ability to provide rapid inhaled bronchodilator therapy by nebulizer is important for quick relief of obstructive airway symptoms in emergency care and the general hospital environment. In situations where breath actuated delivery of nebulized medication is not available, constant output jet nebulizers are routinely used. This study compared three commonly used continuous jet nebulizers in US hospitals and assessed the drug delivery and speed of delivery. METHODS: Devices tested were MC 300 (Monaghan Medical), Misty Max 10 (AirLife) and Misty Fast (AirLife). Each nebulizer group (n=5) was operated with compressed wall air set to 8 L/min and with a 3-mL fill volume (833 µg/mL albuterol sulfate). A breathing simulator was set to mimick adult (and child) tidal breathing patterns: tidal volume = 500 (155) cc, rate/min = 10 (25) cycles, inspiratory: expiratory ratio =1:2. A filter was replaced minute-by-minute at the mouthpiece to capture the total mass until sputter (TM), sputter being considered representative of treatment time. Albuterol recovered was quantified by UV-spectrophotometry. In parallel, respirable droplet fraction <4.7 mm (RDF<4.7 mcm) was determined by laser diffractometry. The clinically relevant respirable droplet mass (RDM<4.7 mcm) was calculated as TM x RDF<4.7 mcm and represented as mean and SD in the results. RESULTS: The MC 300 nebulizer produced RDM<4.7 mm values of 185.7mcg ± 10.2 and 304.0mcg ± 32.3 for the child and adult breathing patterns respectively. In comparison, the Misty Max 10 produced 171.3mcg ± 6.9 and 227.0mcg ± 50.1, and the Misty Fast produced 125.6mcg ± 4.8 and 182.1mcg ± 23.7 The treatment times to sputter for the MC 300 and Misty Fast devices both ranged from 3 to 4 mins whereas the Misty Max 10 ranged from 4 to 5 mins. CONCLUSIONS: The MC 300 nebulizer produced the highest respirable doses (for both adult and child breathing patterns) and the equal best (with Misty Fast) speed of delivery. The Misty Max 10 device was second in terms of drug delivery but had longest delivery time. CLINICAL IMPLICATIONS: These results provide clinicians with data to support an informed selection of nebulizer in cases where the goal of maximizing lung delivery with shortest treatment time is desired.
