• W4387249128 abstract "SESSION TITLE: Chest Infections Posters 9 SESSION TYPE: Original Investigation Posters PRESENTED ON: 10/11/2023 12:00 pm - 12:45 pm PURPOSE: Retrospective data was used to compare in-hospital mortality of adult patients diagnosed with COVID-19 infection, with and without underlying pulmonary hypertension. METHODS: Adult patients admitted with COVID-19, with and without pre-existing pulmonary hypertension (PH) were analyzed from January 2020 to December 2020 using the National Inpatient Sample database. SAS 9.4 software was used to run univariate and multivariate analysis to determine baseline characters, Elixhauser comorbidities and to obtain outcomes. The primary outcome was in-hospital mortality. Secondary outcomes included length of stay (LOS), total hospital costs and disposition. RESULTS: Among patients hospitalized with COVID-19, 42,075 (2.5%) patients with underlying PH (group A) and 1,618,025 (97.5%) without PH (group B) were analyzed. Group A consisted of 45.3% males and 54.7% females. Mean age of patients in group A was 70.9 ± 14.4 compared to 63 ± 17.6 in group B. Patient population was predominantly caucasian in both groups. Comorbidities including hypertension, coronary artery disease, congestive heart failure, COPD, peripheral vascular disease, renal failure, diabetes mellitus and obesity were higher in group A compared to group B. In-hospital mortality was higher (22.9% vs.13.2%, P<0.001) in group A. The mortality adjusted odds ratio (OR) was 1.18 (95% confidence interval 1.15 – 1.21 P<0.0001). Patients in group A also had a longer length of stay (10.5 ± 10.7 vs. 7.9 ± 9.3, P<0.0001), with higher cost of hospitalization (30,816 ± 53,775 vs. 21,354 ± 38,318, P<.0001), and increased need for inpatient rehab upon discharge (26.2% vs.18.3%) CONCLUSIONS: In patients hospitalized with COVID-19, underlying PH resulted in increased in-hospital mortality, higher burden of cost, longer length of stay and the need for inpatient rehab post hospital discharge. CLINICAL IMPLICATIONS: Patients with a history of PH admitted with COVID-19 are at higher risk of in-hospital mortality. Specific treatment modalities for COVID-19 in patients with underlying PH is an area to be explored. Further research is needed to implement a clinical protocol to identify patients with pre-existing pulmonary hypertension as they may benefit from early aggressive treatment for COVID 19.
