- W4387249213 abstract "SESSION TITLE: Chest Infections Posters 3 SESSION TYPE: Original Investigation Posters PRESENTED ON: 10/10/2023 12:00 pm - 12:45 pm PURPOSE: COVID-19 patients can develop respiratory failure leading to increased risk of Ventilator-Associated Pneumonia (VAP). This study compares disease severity of VAP among COVID-19 patients and non-COVID-19 patients. METHODS: This is a retrospective cohort study on 602 patients on invasive mechanical ventilation at the intensive care units of a tertiary hospital. Severity outcomes including ICU mortality, length of hospital stay, multiple-drug resistance acquisition, duration to onset of VAP, and microbial organisms isolated were compared for the two VAP groups. RESULTS: COVID-19 patients are ~1.2 times more likely to develop VAP compared to non-COVID-19 patients (OR=1.208, 95%CI=0.964-1.515). COVID-19 VAP patients had higher ICU mortality (59.42% COVID; 40% non-COVID, p=0.011), multiple-drug resistance acquisition (59.42% in COVID; 44.35% non-COVID, p=0.048), longer ICU stay (OR=1.053, 95%CI=1.016-1.092, p=0.004), and longer duration of intubation (OR=1.064, 95%CI=1.006-1.125, p=0.031). Higher prevalence of Gram-negative bacterial isolates was seen in both groups (79.71% COVID; 69.71% non-COVID, p=0.133), which include Klebsiella pneumoniae, Pseudomonas aeruginosa, and Stenotrophomonas maltophilia. More from non-COVID-19 group had early-onset VAP (less than 5 days from orotracheal intubation) (45.22%, p=0.005), while majority of the COVID-19 group had late-onset VAP (5-10 days from OTI) (49.28%, p>0.05); with no significant difference in duration to VAP onset between groups. Diabetes mellitus type 2 (p=0.01), history of antibiotic use in 3 months (p=0.00), and acute respiratory distress syndrome (p=0.00) were seen more in VAP patients with COVID-19. CONCLUSIONS: COVID-19 infection increases severity of patients with ventilator-associated pneumonia in terms of higher ICU mortality, increased acquisition of multiple-drug resistance, longer ICU stay and duration of intubation. CLINICAL IMPLICATIONS: The presence of COVID-19 infection can add to the already high risk, numerous adverse outcomes of ventilator-associated pneumonia. It helps to be able to anticipate disease severity, possible microbial isolates, and risk in acquiring multiple-drug resistance to aid clinicians in the management of these patients.
