Evidence suggests that the level of urinary desmosines (uDES) is associated with emphysema severity and exacerbations in patients with chronic obstructive pulmonary disease (COPD). The main objective of the present study was to investigate the association between uDES and long-term mortality in patients with COPD. METHODS: A total of 180 participants were selected from the Korean Obstructive Lung Disease (KOLD) cohort. Urine samples were assayed for desmosines using liquid chromatography-tandem mass spectrometry (LC-MS/MS). The participants were divided into four quartile groups based on their baseline uDES concentrations: lowest, low, mid, and high uDES. RESULTS: Statistically significant differences were observed among the four groups in Charlson comorbidity index, body mass index (BMI), forced expiratory volume in 1 s (FEV1), diffusing capacity, and 6-min walk distance. The mortality rates were significantly higher in the high uDES group than in the other groups during the 161-month follow-up period. Kaplan-Meier analyses also revealed that participants in the high uDES group survived for a significantly shorter duration than those in the other groups (p<0.001). Multivariate Cox proportional hazards regression analysis revealed that participants in the high uDES group demonstrated an approximately five-fold increased risk of death (adjusted HR, 4.86; 95% CI, 1.35-17.46; p = 0.016). CONCLUSIONS: Patients with COPD with high baseline levels of uDES demonstrated significantly higher mortality than those with lower uDES. High uDES level is independently associated with an increased risk of long-term mortality in COPD patients. CLINICAL IMPLICATIONS: There are no reports on the relationship between urinary desmosines and long-term mortality in patients with COPD. The present study showed that patients with COPD in the high urinary desmosine group showed significantly poorer survival than those in other groups during a median 9.5 years of follow-up, and had an approximately five-fold increased risk of mortality.
