It is essential to promptly diagnose this pathology with Computed Tomography angiography and promptly treat it to avoid sepsis and potentially death. Cardiac arrythmias, such as atrial fibrillation, are often the culprit of acute ischemic disorders of the intestine. This study aims to identify the outcomes of cardiac arrythmias in patients with acute ischemic disorders of the intestine for better awareness and prevention for future care. METHODS: We reviewed the National Inpatient Sample database from 2016 to 2018. We used the International Classification of Diseases, Tenth Revision, Clinical Medication (ICD-10 CM) discharge codes to identify adults’ patients with acute ischemic disorders of the intestine. These patients were then divided into two cohorts: those who had cardiac arrythmias and the ones who did not. Cardiac arrythmias include supraventricular tachycardia, atrial fibrillation, atrial flutter. We examined the independent effect of the diagnosis of cardiac arrythmias on the risk of acute ischemic disorders of the intestine by using a multivariable logistic regression model controlling for multiple confounders. RESULTS: We identified 27,764 patients with a diagnosis of acute ischemic disorders of the intestine. The mean age was 67.41 ± 15.20 years, and most patients were female (59.02%) and Caucasian (76.44%). The overall in-hospital mortality was 5,426 patients (19.57%), average cost for hospitalization was $147,965 (IQR $31,142-$154,705), and the average length of stay was 9.86 ± 13.44 days. Cardiac arrythmias were noted in 26.94% of patients with the most common being atrial fibrillation and atrial flutter, 23.48%. The development of cardiac arrhythmias was associated with a higher in hospital mortality of 30.35% for the group with cardiac arrythmias, higher hospitalization costs $189,258 versus $132,775 for those without cardiac arrythmias, and higher length of stay for the group with cardiac arrythmias, 11.72 compared to 9.17 days, p-value <0.0001. Predictors of cardiac arrythmias, which were all associated with a p-value <0.0001, were male sex with (odd ratio (OR) of 1.166 and 95% Confidence Interval (CI) of 1.10-1.24), type 2 diabetes (OR of 1.32 and 95% CI of 1.23-140), obstructive sleep apnea (OR of 1.55 and 95% CI of 1.38-1.73), COPD (OR 1.49 and 95% CI of 1.39-1.60), history of cardiac arrest (OR of 1.91 and 95% CI of 1.17-3.12), hyperkalemia (OR 1.36 and 95% CI of 1.23-1.51). CONCLUSIONS: The results of our analysis demonstrate a higher prevalence of cardiac arrythmias in patients with acute ischemic disorders of the intestine. Cardiac arrythmias were also associated with a higher case of mortality, length of stay, and cost of hospitalization in patients with acute ischemic disorders of the intestine. CLINICAL IMPLICATIONS: It is essential to effectively diagnose and treat cardiac arrythmias to prevent complications that may result from them such as acute ischemic disorders of the intestine. Complaints of acute onset abdominal pain in patients with cardiac arrythmias should raise concerns for acute ischemic disorders of the intestine.
