- W4387252194 abstract "SESSION TITLE: Nurturing Growth: Fostering Success in Trainees SESSION TYPE: Original Investigations PRESENTED ON: 10/10/2023 01:45 pm - 02:45 pm PURPOSE: Physician residents during the COVID-19 pandemic witnessed immense amounts of death and morbidity. During the peak of the Omicron surge, ICU residents at Ascension St. Vincent’s Indianapolis Hospital had as many as four comfort care planning discussions per day with family members, leading to over 50% of all extubations resulting in terminal weans. Guiding patients and families through these goals of care discussions has a significant impact on residents’ emotional and psychological well-being. To date, there is scant research on improving resident well being that is also compatible with the complexities of the current resident schedule. METHODS: We developed a monthly recurring faculty-free, time protected group session during work hours for residents to confidentially discuss events, situations, or feelings related to their time in the ICU without judgment or repercussion. A facilitator from Ascension Ministry Formation was present to aid in facilitating conversation. The first ICU resident support group session was held in late February 2022 by having residents use pictures (e.g. hurricanes, busy highways, etc) as allegorical tools to connect to their experiences in the ICU. After each session we collect feedback anonymously from residents to continue to improve future sessions. RESULTS: In answering the question, “What are some reasons that you chose to share your experiences today”, 70% of participants wished to hear other participants' perspectives. 40% wanted help in processing a difficult experience. After the discussion, 70% of residents noted that they felt more connected to their fellow residents/interns. 50% of residents felt that they were able to provide better patient care. 100% of participants responded that they would recommend other residents to participate in this discussion. CONCLUSIONS: Physician residents are tasked with a tough role as frontline care providers, especially within the scope of the pandemic. Providing a space with a facilitated discussion format using narrative processing has the potential to improve resident well being and ultimately reduce resident burnout and improve patient care. CLINICAL IMPLICATIONS: Multiple studies have documented up to a 50% prevalence rate of resident burnout. Residencies nationally have created wellness programs to address sleep, fitness, and mindfulness; however, these programs typically take place outside of work hours which cuts into residents’ already limited time away from the hospital. To date, there are very few studies in the literature on the effectiveness of these programs or how to specifically prevent emotional burnout. We believe that residency sponsored wellness programs that target emotional well being are critical to help create resilient and empathetic resident physicians.
