- W4387260096 abstract "The aim of this study was to determine whether measuring fetal fibronectin (fFN), cervical length and maternal blood inflammatory markers in isolation or combination is useful in predicting impending delivery in twin pregnancies with preterm labour and intact membranes. This retrospective cohort study included 121 consecutive twin gestations with preterm labour and intact membranes (22-34 weeks). At the time of admission, vaginal fluid was collected and a quantitative fFN test with a cut-off of 50ng/mL was performed. Additionally, cervical length, maternal white blood cell count (WBC), and C-reactive protein (CRP) concentration were measured. Primary outcome measure was delivery within 7 days of the test. 1. The rate of spontaneous preterm delivery within 7 and 14 days were 9.1% (11/121) and 14.9% (18/121), respectively; 2. A positive fFN result was significantly associated with delivery within 7 days (P = 0.001), while cervical length and gestational age showed a trend but without statistical significance (P < 0.1); 3. Maternal WBC count and CRP concentration did not differ between patients who delivered within 7 days and those who did not (P > 0.1); 4. Multiple logistic regression revealed a positive fFN test was significantly associated with delivery within 7 days (P = 0.01); 5) In multivariate survival analysis, positive fFN tests and advanced gestational age were significantly associated with a shorter measurement-to-delivery interval (P < 0.05 each). A positive fFN test was found to be the most effective predictor of impending delivery in twin pregnancies with preterm labour and intact membranes, whereas cervical length, maternal WBC count, and CRP concentration measurements alone or in combination were not useful in predicting delivery within 7 days." @default.
