- W4387260438 abstract "Abdominal congenital masses are very rare, with an incidence of 2-14 newborns per 100,000 livebirths, though there are likely many unreported cases as well. We present the case of an obese 26 years old primigravida diagnosed with GDM recurrent genito-urinary tract infections, showing in the second trimester ultrasound scan an oval hypoechogenic mass localised in the right hemiabdomen measuring 4/5 cm, with no Doppler signal, thick wall and little peristalsis. Fetal MRI raised the differential diagnosis of choledochal cyst, mesenteric or omental cyst, intestinal duplication cyst and duodenal atresia – excluded by ultrasound findings: normal appearing stomach and amount of amniotic fluid. The cystic mass maintained its ultrasound characteristics throughout pregnancy and increased its size at 6/7 cm at birth. The fetus presented normal biophysical profile and AFI throughout pregnancy, with EFW above 90th centile and was delivered at 39 weeks of gestation by Caesarean section (male neonate, weighing 4200g, Apgar score 8). Postpartum abdominal US showed a hypoechogenic structure occupying the entire right hemiabdomen, with small hyperechogenic foci and without peristalsis. Neonatal prognosis was good. Pediatric surgery consult in the 3rd day of life established the differential diagnosis between mesenteric/omental cyst and intestinal duplication cyst. Surgery was performed after 2 weeks confirming an enteric duplication cyst. Immediate and 6 months postoperative prognosis was favourable. Our case underlines the challenges in differential diagnosis of fetal abdominal cystic masses, especially in case of obese patients.
