- W4387260500 abstract "Transvaginal ultrasound is the mainstay of imaging of the gynecological organs. Imaging of the non-gynecological structures is also being enhanced during routine pelvic scanning. Pelvic part of the ureters are frequently involved in endometriosis, fibroids and gynecological malignancy. Imaging of ureters has been described in parasagittal plane. However, it can be challenging. Therefore, the aim of this study was to provide an easier approach to assess the distal ureters by using power Doppler. 200 patients with normal uterus and ovaries were included. TVS was performed with partially full bladder. The transverse view of the urinary bladder was obtained with the probe placed in vaginal fornix. Probe was slightly withdrawn till we see the trigone of the bladder. Use of power Doppler lighted up the origin of the ureteric jets arising from the ureterovesical (UV) junctions. The ureteric jets were spotted at the lateral aspect of the trigone between 10 and 2 o'clock position. PRF setting of 0.3-0.6 kHz was used. Spotting the UV junctions by power Doppler aids in tracing the distal ureters. Power Doppler spotted Ureteric jets in 97.5% (195 patients) on transvaginal ultrasound with partially full bladder. 3 patients showed unilateral absence of ureteric jets on power Doppler due to presence of calculi in the lower ureter. 2 patients had technical limitation of visualising the ureteric jets. This method of evaluating ureters appears less challenging than the established parasagittal method for evaluating ureters. Power Doppler lights up the ureteric jets which are seen along the trigone of the partially full urinary bladder and which serves to spot the UV junctions. This approach is simple and easier for the examination of ureters on routine pelvic imaging.
