- W4387260584 abstract "The fetal weight estimated on ultrasound very often influences obstetric care and decisions. But several parameters can influence this estimate, including maternal obesity. The aim of this study is to evaluate the accuracy of the ultrasound estimation of the fetal weight in patients at the entrance to the delivery room according to the body mass index (BMI) of the patients. It is a retrospective case-control study including all the patients that had their 3rd trimester ultrasound while in labour fin our delivery room during the year 2022, comparing the accuracy of the ultrasound estimation of fetal weight (EFW) based on women's BMI relative to birthweight (BW). Our study population is divided into two groups according to BMI (20 to 29.99 and greater than or equal to 30). For each BMI group, the accuracy of ultrasound estimation of fetal weight relative to birthweight was calculated. Each study group included 180 patients. The two groups were comparable regarding maternal characteristics except for BMI. The median absolute difference between EFW and BW was 287 [0-1250] grams. The median percentage of error was 7% [0 to 38%]. This margin of error was less than 10% in 72.5% of cases. There was a good correlation between EFW and BW (R= 0.7). This correlation was stronger in the normal BMI group (R = 0.76 versus R = 0.68; p = 0.03). A BMI≥30kg/m2 altered significantly the performance of the ultrasound estimation with a percentage of error of 8.9% versus 7.4% in the 20<BMI<30kg/m2 group (p = 0.02; 95% CI; [0.47-0.89]). In addition, a BMI≥30 kg/m2 was associated with a greater number of non-detected macrosomia on the ultrasound estimation, i.e. 8.7% of false negatives (FN): Indeed, among FN, 80% had a BMI≥30 kg/m2. The negative predictive value was 93.6% good but more impaired in the BMI≥30 kg/m2 (88.6% versus 95.4%, p = 0.001; 95% CI, [0.75-0.86]) Maternal obesity alters the performance of ultrasound in the delivery room, which should not be the only obstetric decision-making aid, but should be associated with the clinic." @default.
