- W4387260609 abstract "Assess the performance of different Endometrial Thicknesses (ET) to detect Endometrial Cancer and Hyperplasia (ECH) in postmenopausal bleeding (PMB). Prospective study of 1635 women 50 years and older who underwent endometrial evaluation between 2/2014-10/2019. Outcome data was obtained through 1/2021. Transvaginal sonogram (TVS) and endometrial aspiration (EA) data was collected. All cases with surgical outcome confirmation and/or benign follow-up > 1 year were included. Endometrial thickness (ET) thresholds of > 3, 4 and 5 mm were used as positive TVS. Sensitivity, Specificity, Positive Predictive Value (PPV) and Negative Predictive Value (NPV) of 3, 4 and 5 mm ET were analysed. 1170 women were included in the analysis. Mean age was 59.7 years and mean BMI 33.6. 736 (62.9%) had surgical confirmation and 434 (37.1%) had benign EA and uneventful follow-up > 1 year. 124 (10.6%) had ECH outcome including 107 “current ECH” and 14 ECH cases were diagnosed after > 1 year. Table 1 depicts the methods' performance in detection of “current ECH” (1A) and “all ECH” (1B) (including the cases diagnosed after > 1 year). In “Current ECH”: The Sensitivity and NPP of 3- and 4 mm ET were similar (p = 0.16) and were higher than 5 mm (P =< 0.001). 4 mm ET had better Specificity (P < 0.001) and PPV (P < 0.001) than 3 mm. Analysis of “All ECH” and “cancer only” (hyperplasia excluded) showed similar results. 3 and 4 mm TVS ET had similar excellent performance of excluding ECH, with 4 mm having significantly better specificity and PPV.
