- W4387262804 abstract "Triplet multiple pregnancies are associated with an important increase in maternal risks and neonatal morbidity. The condition is even more complex when the fetuses share chorionicity and amnionicity. In addition to the possibility of clinical, maternal, and fetal complications, it often also represents an ethical challenge when there is a need for interventions and/or early interruption for the benefit of one of the triplets. We report the case of a 39-year-old pregnant woman referred to the Fetal Medicine Service for a dichorionic-triamniotic triplet pregnancy complicated by fetal death of one of the shared placenta fetuses diagnosed at second trimester scan. Our ultrasound assessment by 25 weeks showed that fetal demise may has occurred by 16 weeks estimated by biometry of fetus 1, matching with maternal infection by COVID-19, and showed fetal growth restriction of the shared diamniotic-monochorionic fetus (fetus 2), associated to microcephaly due to cortical atrophy resulted from cerebral hypoxic-ischemic event associated to the fetal death of the fetus 1. The third fetus (fetus 3) from different chorionicity showed a normal ultrasound development and fetal weight. Due to religion and beliefs, parents decided to continue the pregnancy. Pregnancy was complicated by severe pre-eclampsia and delivery occurred at 32 weeks and 1 day due to maternal decompensation, with the birth of two live fetuses under the care of the Neonatal Intensive Care Unit team. Fetus 3 was born with the weight of 1620 gram and had an Apgar score of 4/8. Fetus 2 weighed 1200 gram and had an Apgar score of 3/7. The dead fetus 1 weighed 365g with papyraceous aspect. Placenta analysis confirmed dicoriocinity and shared placenta of fetus 1 and 2 with near insertion of cord vessels. The characteristics of the reported case led to the discussion of criteria for deciding the best time to delivery in such distinguish fetus situation with the purpose of better neonatal outcomes." @default.
