- W4387263303 abstract "Accurate prediction of the time to delivery (TTD) is an important clinical significance in the obstetric field. The objective of this study is to predict the time to delivery through the rate of increase in angle of progress with ultrasound. 102 nulliparous women at ≥37 weeks of gestation with singleton with spontaneous labour or induction of labour in cephalic presentation were included. A measuring angle of progress (AOP) with transperineal ultrasound was performed by a separate examiner within 10 minutes. Time to delivery was determined from the active phase to delivery of the fetus. In descriptive statistics, categorical variables were analysed with Fisher exact test and continuous variables were compared with t test and 1-way analysis of variance. Interaction analysis between variables and AOP was performed by using linear regression and Pearson correlation coefficient. Interobserver agreement was presented with the intraclass correlation coefficient and with limits of agreement with a Bland–Altman plot. The mean TTD value was 205.78 ± 71.75 minutes and this value according to AOP could be presented as an exponential function (TTD_value=exp (21.28-0.14*AOP)). In exponential model of interaction analysis, epidural anesthesia, occipitoposterior fetal position and amniotic fluid index did not affect TTD value with AOP. However, TTD was longer at the same AOP value in oxytocin infusion group and shorter in PROM group. Neonate body weight, height, head and thoracic circumference was not correlated to TTD with AOP. The AOP is objective and accurate method for predicting TTD in nulliparous women after active phase.
