- W4387263753 abstract "Congenital abnormalities of the kidney and urinary tract (CAKUT) are one of the leading fetal anomalies seen on an ultrasound during pregnancy. Antenatal diagnosis of a CAKUT represent 20-30% of all major birth defects. 2D ultrasound is the initial modality for prenatal diagnosis of fetal anomalies, however colour flow Doppler plays an additional role. A 20-year-old gravida 1 para 0 pregnant patient at 18 4/7 weeks presented for obstetric ultrasound utilising a Philips EPIQ Elite with a C5-1 curvilinear and V9-2 PureWave volume transducers. The ultrasound demonstrated a breech, female fetus with normal estimated fetal weight. The fetal anatomic survey identified anhydramnios and bilateral multicystic dysplastic kidneys. The patient was enrolled in a study of Flow Viewer which was ongoing at our practice. Colour flow with Flow Viewer, designed to enhance colour with sharp delineation of vascular margins and 3D-like rendering was utilised to assess the fetal renal anatomy. The patient returned at 35 5/7 weeks and obstetric ultrasound demonstrated a vertex fetus with fetal growth restriction. Re-evaluation demonstrated anhydramnios, however the previously visualised multicystic dysplastic kidneys could not be identified on 2D imaging raising concern for bilateral renal agenesis. Colour flow imaging was suboptimal, however was suggestive of no evidence of renal arteries. Using Flow Viewer, the fetal aorta was identified, clearly demonstrating bilateral absence of the renal arteries. The patient was informed of the findings of bilateral renal agenesis and the guarded prognosis. In this case, the diagnosis was not conclusive using 2D imaging and colour flow. The diagnosis of bilateral renal agenesis progressing from bilateral multicystic dysplastic kidneys was enhanced using Flow Viewer. This new colour Doppler imaging modality allowed for superior imaging of the fetal aorta and the ability to definitively demonstrate complete and bilateral absence of the fetal renal arteries." @default.
