- W4387263807 abstract "To assess the uteroplacental perfusion characteristics of monochorionic twin pregnancies complicated by different types of selective growth restriction by using three-dimensional power Doppler (3DPD). A prospective cross-sectional study was conducted for 155 monochorionic-diamniotic twin pregnancies with sGR and 155 normal twin pregnancies in a tertiary referral centre. In twins with sGR, 104 were type 1, 31 were type 2, and 20 were type 3. Uteroplacental perfusion parameters including the vascularisation index (VI), flow index (FI), vascularisation flow index (VFI) in co-twins were assessed by 3DPD. Traditional Doppler parameters were acquired at the same time. The discordances of VI, FI, and VFI in type 1 and 2 were higher than those in controls; the discordances of FI in type 3 were lower than those in type2. For the parameters of the larger fetuses, FI in type 3 were lower than those in controls and type 1. For the parameters of the smaller fetuses, VI and VFI in type 1, 2 and 3, and FI in type 1 and 2 were lower than those in controls. VI in type 2 was lower than that in type 1. There were positive correlations between the GA at ultrasound scan and FI of the larger fetuses. There were significant correlations between the discordances of EFW and the discordances of VI, FI and VFI, as well as VI, FI and VFI of the smaller fetuses. In monochorionic twin pregnancy with sGR, the numbers of placental blood vessels of the smaller fetuses decreased in all types, but the blood volumes only decreased in type 1 and 2. Type 3 showed an increase, rather than decrease, of FI in the smaller fetuses. Moreover, the discordances of FI appear to be the only indicator that could distinguish type 3 from type II. Our findings suggested that type 3 sGR showed special characteristics of utero placental perfusion, which was closely associated with its special pathological mechanism." @default.
