- W4387264384 abstract "To compare the combination of oxytocin plus balloon (O-B) versus misoprostol plus balloon (M-B) for induction of labour (IOL) in high-risk pregnancies. This was a single-centre, open-label RCT. Individuals with high-risk pregnancy, at least ≥ 22 weeks', with a singleton in cephalic presentation, Bishop score ≤ 6, and intact membranes were offered enrolment. High-risk pregnancy was defined by the presence of any of the following: hypertensive disease of pregnancy, FGR (estimated fetal weight or abdominal circumference < 10th percentile), oligohydramnios (deepest vertical pocket < 2cm), suspected placental abruption requiring delivery, uncontrolled pregestational diabetes, or abnormal biophysical profile or non-stress test requiring delivery. The primary outcome was the rate of Caesarean delivery (CD). An intent-to-treat analysis was performed. This RCT was IRB approved and registered on ClinicalTrials.gov (NCT04492072). From August 2020 to May 2022, a total of 150 individuals were randomised, (49% assigned to OB and 51% with MB). There were no differences in baseline characteristics. The most common indication for IOL was FGR, followed by gestational and chronic hypertension. There was no significant difference in the rate of overall CD between pregnancies undergoing IOL with O-B compared with M-B, (21.9% vs. 31.1%; RR 0.70, 95% CI 0.41-1.21). When focusing on specific indications for CD, there was no difference between IOL with OB and MB for either arrest disorder or failed induction (68.7% vs. 45.8%; RR 1.50. 95% CI 0.87-2.59) and NRFHR (31.3% vs. 45.8%, RR 0.68, 95% CI 0.29 – 1.59). There were no differences in secondary maternal outcomes including uterine tachysystole rates, total IOL to delivery time, vaginal births within 24 hours, or postpartum hemorrhage rates. There were also no differences in secondary fetal/neonatal outcomes. In high-risk pregnancies, the rate of CD and other adverse maternal and fetal outcomes was similar for induction of labour with OB compared with MB." @default.
