- W4387268434 abstract "SESSION TITLE: Severity and Outcomes of Disease in Pulmonary Vascular Disease SESSION TYPE: Original Investigations PRESENTED ON: 10/10/2023 01:45 pm - 02:45 pm PURPOSE: Acute respiratory distress syndrome (ARDS) is characterized by diffuse alveolar damage resulting in hypoxic respiratory failure. Pulmonary hypertension (PH) is a recognized consequence of ARDS and likely occurs due to multiple mechanisms, including pulmonary vasoconstriction, thromboembolism, and interstitial edema. Furthermore, it is likely that the magnitude of PH correlates with the severity of lung injury in these patients. As a result, we hypothesized that the presence of PH in ARDS patients is a predictor of worse outcomes. METHODS: We conducted a retrospective analysis of hospitalizations with a primary diagnosis of ARDS using the NIS database from 2016 to 2018. We included adult patients (≥ 18 years) with a diagnosis of ARDS with and without coexisting PH using the International Classification of Diseases, Tenth Revision, Clinical Modification (ICD-10-CM) codes. We compared the outcomes of these two groups, including in-hospital mortality, length of stay (LOS), and total hospitalization costs. We also performed multivariate logistic and linear regression analyses to examine the association between PH and ARDS outcomes and adjust for potential confounding factors such as age, gender, comorbidities, and hospital characteristics. RESULTS: We identified 156,687 hospitalized patients with ARDS, of which 16.8% (n=26,324) had a diagnosis of PH. Compared to patients with ARDS alone, those with coexisting PH were older, more likely to be female, and had greater comorbidities. Individuals with ARDS and coexisting PH had significantly higher rates of in-hospital mortality (36.8% vs. 24.6%), a longer length of stay (12 vs. 10 days), and higher total hospital charges ($210,165 vs. $160,683). After adjusting for confounding variables, those with ARDS and coexisting PH had a higher odds ratio (OR) of in-hospital mortality (OR 1.52, 95% CI 1.46-1.58, p<0.001), a longer LOS (OR 1.37, 95% CI 1.35-1.38, p <0.001), and higher total hospital expenditures (mean difference $19,406, 95% CI $18,482-$20,329, p<0.001). CONCLUSIONS: Our study suggest that patients with ARDS and coexisting PH have worse outcomes than those with ARDS alone. Furthermore, the presence of PH may be a surrogate for disease severity in ARDS patients. Clinicians should be aware that PH in hospitalizations with ARDS may be associated with worse outcomes, including higher mortality rates, longer hospital stays, and higher healthcare costs. CLINICAL IMPLICATIONS: The findings of our study highlight the importance of early identification and management of PH in patients with ARDS, which could potentially reduce mortality rates, shorten hospital stays, and decrease the cost of care. Further research is needed to explore the potential benefits of PH-targeted therapies and optimal management strategies for patients with ARDS and PH. Our study provides important insights into the outcomes of hospitalizations with ARDS and PH and contributes to the growing body of literature on this topic.
