One of the most serious problems of the elderly is the development of cognitive decline due to cerebrovascular pathology. However, elderly patients often have a large number of comorbid diseases, which leads to difficulties in diagnosing and managing these patients, and often to the development of polypharmacy, which can lead to deterioration in functional status, cognitive impairment, adverse reactions and drug interactions. In addition, in elderly patients, there may be changes in pharmacokinetics and pharmacodynamics due to anatomical and physiological involutive processes. At the same time, the number of drugs whose clinical efficacy and tolerability were evaluated specifically in elderly and senile patients is relatively small. In a randomized clinical trial of sequential parenteral and oral therapy with Mexidol in patients with mild vascular cognitive impairment syndrome, a positive effect of this therapy on various domains (cognitive, emotional, autonomic, motor) of chronic cerebrovascular disease was confirmed compared with placebo, which allows us to recommend it for use in elderly and senile patients.Проблема фармакотерапии лиц пожилого и старческого возраста в настоящее время является крайне актуальной в связи со старением населения и ростом распространенности сердечно-сосудистых заболеваний. Одной из серьезнейших проблем пожилого человека является развитие когнитивного снижения, обусловленного цереброваскулярной патологией. Однако у пожилых больных часто отмечается большое количество коморбидных заболеваний, что приводит к трудностям диагностики и ведения этих пациентов, а зачастую — к развитию полипрагмазии, которая может привести к ухудшению функционального статуса, когнитивным нарушениям, побочным реакциям и лекарственным взаимодействиям. Кроме того, у пожилых пациентов могут наблюдаться изменения фармакокинетики и фармакодинамики, обусловленные анатомо-физиологическими инволютивными процессами. В то же время препаратов, эффективность и переносимость которых оценивались именно у пациентов пожилого и старческого возраста, относительно немного. В рандомизированном клиническом исследовании последовательной парентеральной и пероральной терапии Мексидолом у пациентов с синдромом умеренного когнитивного расстройства сосудистого генеза было подтверждено положительное влияние указанной терапии на различные домены (когнитивный, эмоциональный, вегетативный, двигательный) хронического цереброваскулярного заболевания, что позволяет рекомендовать его для применения у больных пожилого и старческого возраста." @default.
