- W4387396029 abstract "Dating pregnancies plays a central role in antenatal care. Growth charts for ultrasonographic early pregnancy variables exist, but widely used ones have been developed decades ago: Gestational age (GA) determined by the crown-rump length (CRL) 1975 by Robinson and Fleming (1) and 1992 by Hadlock (2). The aims of our study were: 1) Create updated reference charts for CRL and gestational sac diameter (GSD) for first-trimester singleton pregnancies of spontaneous and assisted reproductive technology (ART) -conceived pregnancies. 2) Compare biometric growth curves of spontaneous and ART-conceived gestations. 3) Compare the novel growth charts based on exact implantation dates with preexisting charts created based on last menstrual period (LMP) dating. 1072 healthy singleton live births between Jan 2010 and Dec 2020 from one university fertility clinic were included. Growth charts were created using longitudinal data from a median of 4 first trimester visits at different gestations. CRL measurements for exactly dated ART gestations and for spontaneous pregnancies dated by LMP in women with regular cycles were plotted according to gestational days. Quantiles of CRL dependent on GA were estimated using an ordinal regression model (proportional odds model). The dependency of CRL on GA was modeled in a nonlinear fashion using a restricted cubic spline. Novel reference charts for ultrasound dating of GA based on CRL and GS were created. While the 50th percentile of the CRL measurements was similar in spontaneous and ART pregnancies, the difference of the 10th and 90th percentile (“spread”) was statistically significantly higher in spontaneous compared to ART pregnancies. While the pre-existing charts were largely consistent with our newly created ones, they overestimate the GA by 1-2.5 days at 11 weeks when compared to our novel reference curve created by exact ART timing. GA estimation by CRL is associated with a more than two-fold higher spread in spontaneous compared to ART-conceived gestations.
