- W4387396265 abstract "To evaluate whether seasonal variability influences characteristics of fertility patients who received methotrexate (MTX) as treatment for pregnancies of unknown location. Previous studies suggest that season and temperature may impact embryo quality and fertilization rates, and that environmental or lifestyle factors more prevalent in the late summer may be associated with increased risk of early spontaneous abortion (1, 2). The impact of seasonality on patients with abnormal bHCG trends in early pregnancy has not been specifically addressed to date. This was a retrospective cohort study of all fertility patients from December 2019 – January 2023 who received MTX to manage abnormally trending bHCG with pregnancies of unknown location including either confirmed ectopic pregnancy or presumed ectopic pregnancy. The primary outcome was the number of abnormal pregnancies requiring MTX across all seasons. ANOVA and Chi Square analyses were used to compare the four seasons. Of 105 patients in this cohort, 26% (27/105) received MTX in the Winter, 21% (22/105) in Spring, 25% (26/105) in Summer, and 29% (30/105) in Fall; differences between seasons were not statistically significant (p=0.93). There was a statistically significant difference among types of cycles included in this study with more frozen embryo transfers (FET) and ovulation induction (OVU) cycles when compared to natural/intrauterine insemination (NAT/IUI) cycles (p=0.01). Furthermore, there were statistically fewer FET cycles requiring MTX in the Summer when compared with other seasons (p=0.04). NAT/IUI and OVU cycles had similar rates of MTX administration across all seasons. There was no statistically significant difference in age (p=0.37), AMH (p=0.71) and cycle day on which MTX was administered (p=0.78), initial bHCG (p=0.93), or progesterone levels (p=0.70) among all four seasons (Table 1). The mean number of fertility patients with abnormal bHCG trends that received MTX did not vary by season. Rates of abnormal bHCG trends and subsequent MTX administration are stable throughout the year. However, fewer FET cycles required MTX in the Summer when compared with other seasons.
