- W4387396284 abstract "To determine identifiable risk factors that lead to an increased likelihood of restarting androgenic anabolic steroids (AAS) in effort to inform shared decision-making in men using AAS while simultaneously seeking fertility treatment. A retrospective review on adult men with infertility due to chronic anabolic-androgenic steroid use between 2012 – 2022. All men were either diagnosed with azoospermia or severe oligospermia and were instructed to stop using AAS. Patients were excluded if there was a history of orchiectomy for benign or malignant conditions. Demographic characteristics were assessed: age, duration of AAS usage prior to fertility assessment, route and dosing of administration, as well as relevant fertility treatments and if AAS was restarted. We hypothesized that risk factors for restarting AAS would include: duration of time on AAS, type of AAS, pre-treatment serum testosterone and increased age. A total of 94 men with azoospermia or severe oligospermia secondary to anabolic steroid use were identified. Of these 94 men, 31 (33.0%) restarted AAS therapy. These men restarted AAS therapy within 8 months (IQR: 6.5-18.0) of cessation. Men who restarted AAS had a median age of 40 years (IQR: 34.5-44.5). Patients that restarted AAS therapy were on AAS for longer duration prior to fertility assessment compared with those who did not resume (60 months IQR: 27-126 vs. 17 months IQR: 4-60, respectively). No statistically significant differences between age, duration of time on AAS therapy, pre-assessment AAS route and dosing as well as the serum testosterone at the time of initial assessment were identified. There was, however, a trend towards statistical significance for duration of time on AAS therapy prior to fertility assessment (p= 0.054). Most men that seek fertility assessment secondary to anabolic steroid abuse did not resume testosterone therapy. Patients that did restart testosterone therapy were on supplemental testosterone for longer periods of time at baseline compared with those who did not. High quality, prospective studies are required to better ascertain risk factors for resuming AAS in patients with male infertility secondary to anabolic steroids." @default.
