- W4387396496 abstract "Investigating man fertility should start with a Spermocytogram (SPG) as a first barrier to explore semen quality, translated through sperm concentration, motility, vitality and morphology. Further analyzes on sperm DNA quality such as of sperm DNA fragmentation (SDF) and Sperm DNA denaturation (SDD) tests might be more informative about the ability of the spermatozoa to fertilize oocytes and to ensure normal embryo development and pregnancy. However, macroscopic characteristics such as of semen viscosity should also be valuable to determine its quality, and should not be ignored compared to other parameters. Therefore, our study focused on the viscosity as a “forgotten” macroscopic characteristic and its ability to inform on SPG as well as SDF / SDD levels. Retrospective study including 10,633 patients (19–68 years old) attending our Laboratory, undergoing SPG for different reasons with 1513 undergoing SDF/SDD testing from January 2013–June 2022. Patients were divided into 2 groups: 9840 patients of Normal Semen Viscosity (NSV) with 1196 patients undergoing SDF/SDD and 793 patients with Higher Semen Viscosity (HSV) including 317 undergoing SDF/SDD tests. Abnormal SDF determined as >30%. Abnormal SDD determined as >30%. Spermocytogram analyzes were performed according to the World Health Organization (WHO) 2010 recommendations using Sperm Class Analyzer software (SCA 5/6 and SCA-scope (Microptic®).Diff-Quick” staining-method coloration was used for morphology analysis. Kit Vita-Eosine (RAL diagnostics®) was used for vitality test. HSV determined when sample leaves the pipette with a drop that forms a thread of >2 cm of length after gentle aspiring (≥30 minutes liquefaction). SDF analyzed with Terminal uridine nucleotide end labeling (TUNEL) and Sperm chromatin dispersion. SDD tested by Aniline Blue staining method. Statistical analyzes performed using SPSS22.0. Kolmogorov–Smirnov for normality and comparisons by Student-t or Mann–Whitney U tests, used as appropriate. Pearson/Spearman’ tests for correlations used as appropriate, P-value<0.05 considered as significant. SDF did not show significant differences between HSV and NSV patients (11% [4 - 24] vs 12% [3 - 35] respectively; p-value=0.671). However, SDD was higher in HSV vs NSV patients (21% [8 - 40] vs 15% [4 - 30] respectively; p-value<0.05). Vitality was decreased in HSV vs NSV patients (38% [0 - 99] vs 24 % [0 - 99%] respectively p=0.01). Correlation analyzes showed negative correlations between Viscosity-Vitality (r= -0.100; p=0.005), Viscosity-SDD (r= -0.091; p=0.003), Vitality-SDD (r= -0.220; p<0.001) and Vitality-SDF (r= -0.214; p<0.001). A negative impact of HSV on Sperm vitality and its DNA integrity through SDD seems to be present. SDD test could be recommended in the presence of HSV. Hypofunction of the prostate / seminal vesicles, Infection and seminal leukocytes, oxidative stress, biochemical and genetic factors could cause HSV. Further investigations are needed to determine which factor has the major impact on semen aspect." @default.
